AI summary — for doctors, generated from the abstract only
Deep endometriosis involving lateral pelvic structures is often underdiagnosed, and MRI with detailed anatomic knowledge is essential for detecting neurovascular involvement and surgical planning.
What was studied
Educational review of lateral pelvic anatomy and MRI findings in deep endometriosis affecting these structures.
Key findings
- Lateral pelvic structure involvement is complex and often underdiagnosed
- MRI is key for detecting lesions and assessing neurovascular involvement
- A multidisciplinary approach is advocated for preoperative staging
Clinical relevance
This supports preoperative MRI by experienced readers when lateral pelvic endometriosis is suspected, such as with sciatic or neuropathic symptoms.
Who should care
Limitations
Educational review without new data.
Endometriosis can reach the pelvic sidewall nerves and vessels, and MRI helps map it before surgery.

Abstract
Deep endometriosis affecting the lateral pelvic structures (LPS) is a complex and often underdiagnosed manifestation of the disease. The authors review the anatomic features of the LPS and underscore the importance of MRI in detecting lesions and assessing neurovascular involvement. A solid understanding of this anatomy is key to accurate diagnosis, surgical planning, and avoiding misdiagnosis. The authors advocate a multidisciplinary approach and highlight MRI as an essential tool for preoperative staging and treatment planning in patients with suspected LPS involvement. ©RSNA, 2026 Supplemental material is available for this article.
