AI summary — for doctors, generated from the abstract only

This review describes permissive cardiotoxicity, a practice approach that accepts some manageable cardiac toxicity to avoid interrupting important cancer treatment.

What was studied

Narrative review outlining the philosophy of permissive cardiotoxicity, a risk stratification framework for cardiovascular toxicities, systems-of-care considerations, and existing literature and guideline recommendations for mitigation.

Key findings

  • The paradigm balances management of treatment-related cardiotoxicity against the risk of interrupting lifesaving cancer therapy.
  • Advances in diagnostic and therapeutic tools have driven the shift.
  • General cardiologists and oncologists, not just cardio-oncologists, need confidence managing cardiotoxicity.

Clinical relevance

Cardio-oncology teams can use shared frameworks to decide when to continue, modify, or pause cancer therapy during cardiac toxicity.

Who should care

Cardiology

Limitations

This is an overview with no quantitative data or described search methods in the abstract, and permissive strategies are an evolving practice rather than a tested standard.

Continuing cancer therapy despite manageable cardiotoxicity is an emerging approach that relies on risk stratification and coordinated care.

Abstract

Permissive cardiotoxicity is an evolving clinical practice pattern that balances the management of cancer treatment-associated cardiotoxicities with the risk of interrupting lifesaving or life-extending cancer treatments. This paradigm shift has been catalyzed by advancements in the diagnostic and therapeutic tools available to treating clinicians, as well as a recognition of the consequences of withholding or delaying cancer therapy. Permissive cardiotoxicity has become a central tenet in the practice of cardio-oncologists; however, recognition and confidence in managing cardiotoxicity is also crucial for the general cardiologist and oncologist due to the growing population of patients with cancer. This review aims to (1) highlight the philosophy of permissive cardiotoxicity, (2) provide a framework for risk stratification of cardiovascular toxicities, (3) describe systems of care considerations for successful implementation of permissive cardiotoxicity, and (4) review the existing literature and guideline recommendations for specific cardiovascular toxicity mitigation.
