AI summary — for doctors, generated from the abstract only

One case report describes histotripsy treating nine of ten liver lesions in a woman with recurrent cholangiocarcinoma without pausing chemoimmunotherapy.

What was studied

A case of a 71-year-old woman with recurrent extrahepatic cholangiocarcinoma and multifocal hepatic metastases progressing on gemcitabine and durvalumab, deemed unsuitable for radiation or other locoregional therapy, treated with histotripsy in a single session.

Key findings

  • Nine of ten hepatic lesions were treated in one session
  • Systemic therapy did not need to be interrupted
  • The authors report short-term local control of oligometastatic progression

Clinical relevance

Suggests histotripsy as a non-thermal, noninvasive option that may fit alongside ongoing systemic therapy in selected patients.

Who should care

Interventional Radiology

Limitations

Single case (Level 4 evidence) with only short-term follow-up and no survival or safety data in the abstract.

An interesting feasibility report, not evidence of benefit.

Abstract

The management of advanced, recurrent extrahepatic cholangiocarcinoma has shifted toward prolonged systemic therapy including immune checkpoint inhibition. Incorporating local treatment for oligometastatic or oligoprogressive disease without interrupting systemic therapy remains challenging. Conventional liver-directed therapies frequently require a chemotherapy break. Histotripsy is a noninvasive, non-thermal, non-ionizing modality that permits continuation of systemic therapy. This case presents a 71-year-old woman with recurrent extrahepatic cholangiocarcinoma who developed multifocal hepatic metastases while receiving gemcitabine and durvalumab. She was deemed not a candidate for further radiation therapy or other locoregional therapies; thus, nine of ten hepatic lesions were treated with histotripsy in a single session. This case highlights the possibility of integrating histotripsy with chemoimmunotherapy by providing short-term local control of oligometastatic progression without the need to interrupt systemic treatment.Level of Evidence: Level 4, Case Report.
